Cleveland Clinic Fairview Hospital is seeking a part-time Care Coordinator to join our multidisciplinary team, focusing on high-risk patients and comprehensive care coordination across the continuum of care.

The role emphasizes patient outreach, education, reassessment, and close collaboration with physicians and clinical staff to promote wellness and optimal outcomes. Flexible schedule with no weekends or on-call requirements.

Qualifications

• Graduate from an accredited school of Professional Nursing.
• Current state licensure as a Registered Nurse (RN).
• BLS Certification through the American Heart Association or the American Red Cross.
• Three to five years of nursing experience.

Responsibilities

• Coordinate care across multidisciplinary teams for high-risk patients.
• Outreach to a defined panel of high-risk patients to ensure timely care delivery.
• Use registries and patient lists to manage populations and assess needs.
• Inform patients and families about care plans and coordinate with team members.
• Reassess progress and update plans of care as appropriate.
• Facilitate transitions of care and remove barriers to access.
• Partner with social work, rehabilitation, pharmacy, and palliative care teams.
• Define disease-specific care paths and assess health literacy.
