Job summary

Fairview Manor is seeking a part-time RNAC (Registered Nurse Assessment Coordinator) to coordinate the MDS and Care Planning process with the interdisciplinary team. You will ensure timely and accurate submissions and support nursing staff education in documentation, MDS, and care planning.

The ideal candidate holds an accredited nursing degree and an active RN license, with CPR recommended. This role emphasizes collaboration, reliability, and delivering quality resident care in a supportive

Qualifications

Graduation from an accredited school of Nursing.
Current license as a Registered Nurse with the State Board of Nursing.
CPR certification recommended.

Responsibilities

Coordinate the MDS and Care Planning process with the Interdisciplinary team.
Complete MDS assessments within established time frames for admissions, quarterly, annual and significant change.
Precheck and electronically submit MDS's to State agency; review validation reports and implement changes.
Schedule and ensure timely completion of MDS's and Care Plans with Case Manager and team.
Participate in the QAPI process and Nurse On-Call program.
Able to access, print and evaluate Quality Indicators.
