HCF is seeking a part-time RNAC (Registered Nurse Assessment Coordinator) in Warren, PA to coordinate the MDS and Care Planning process with the interdisciplinary team. Responsibilities include educating nursing staff on documentation, completing MDS assessments on schedule, prechecking and submitting to the State, and coordinating with the Case Manager.
CPR recommended. Qualifications include graduation from an accredited nursing program and an active RN license with the State Board of Nursing.
Qualifications
- Graduation from an accredited school of Nursing.
- Current license as a Registered Nurse with the State Board of Nursing.
- CPR certification recommended.
Responsabilités
- Coordinate the MDS and Care Planning process with the Interdisciplinary team.
- Assist nursing staff education in documentation, MDS, CAA's and Care Plan process.
- Precheck and electronically submit MDS’s to State agency, review validation reports and make changes.
- Coordinate scheduling and timely completion of MDS’s and Care Plans with the Case Manager.
- Participate in the QAPI process and the 'Nurse On-Call' program.
- Able to electronically access, print and evaluate Quality Indicators.
