Job summary

Gardens at Camphill in Camp Hill, PA seeks a part-time Registered Nurse Assessment Coordinator (RNAC)/MDS Coordinator to join our skilled nursing team. The role focuses on accurate resident assessments, care planning, and regulatory compliance, with a 3-day, 24-hour-per-week schedule and three 8-hour shifts.

The RN is preferred and must have active PA RN license; 1 year MDS/RNAC experience in long-term care is preferred. Competitive pay ranges from $48–$50 per hour.

Qualifications

• Active RN license in Pennsylvania required; RN preferred.
• Minimum of 1 year of MDS/RNAC experience in a skilled nursing or long-term care setting preferred.
• Strong knowledge of the MDS assessment process and long-term care regulations.
• Excellent organizational and communication skills.
• Ability to work independently while collaborating effectively with an interdisciplinary team.
• Strong attention to detail and ability to meet regulatory deadlines.

Responsibilities

• Coordinate and complete resident assessments per federal/state regulations and facility policies.
• Monitor residents for changes in condition and events needing assessment.
• Ensure timely completion of MDS assessments, CAAs, quarterly assessments, and care plans.
• Coordinate with nursing, social services, activities, dietary, therapy, and other departments for accurate information.
• Develop and maintain comprehensive resident care plans based on assessments.
