Gardens at Camphill in Camp Hill, PA is seeking a part-time Registered Nurse Assessment Coordinator (RNAC)/MDS Coordinator to join our skilled nursing team. This role focuses on accurate resident assessments, comprehensive care planning, and regulatory compliance.

This 24-hour-per-week position offers three 8-hour shifts with RN preferred and competitive pay. You will collaborate across disciplines to support timely MDS submissions and high-quality resident care.

Qualifications

• Active RN license in Pennsylvania required; RN preferred.
• Minimum of 1 year of MDS/RNAC experience in a skilled nursing or long-term care setting preferred.
• Strong knowledge of the MDS assessment process and long-term care regulations.
• Excellent organizational and communication skills.
• Ability to work independently while collaborating effectively with an interdisciplinary team.
• Strong attention to detail and ability to meet regulatory deadlines.

Responsibilities

• Coordinate and complete resident assessments in accordance with federal and state regulations and facility policies.
• Monitor residents for changes in condition, hospital returns, significant changes, and other events requiring assessment.
• Ensure accurate and timely completion of MDS assessments, CAAs, quarterly assessments, and care plans.
• Coordinate with nursing, social services, activities, dietary, therapy, and other departments to gather accurate assessment information.
• Develop, update, and maintain comprehensive resident care plans based on assessment findings.
• Maintain and distribute the MDS schedule to the interdisciplinary team.
• Review documentation for accuracy, completeness, and regulatory compliance.
• Submit MDS data to the state as required and maintain accurate supporting records.
• Participate in the interdisciplinary care planning process.
• Ensure assessments and documentation are completed within required timeframes.
