Susquehanna Health & Wellness Center in Columbia, PA is seeking a full-time Registered Nurse Assessment Coordinator (RNAC) to lead our nursing team and oversee MDS assessments. This on-site role requires an active PA RN license, a BSN is preferred, and at least two years as an MDS Coordinator in a skilled nursing facility.

Knowledge of PDPM, Medicare billing, and managed care is essential. The facility offers a 4-star quality environment with robust orientation, benefits, and career growth

Qualifications

• Active RN license in PA.
• BSN preferred.
• Minimum 2 years as MDS Coordinator in a skilled nursing facility.
• Knowledge of rehabilitative and restorative nursing in long-term care.
• Proficient in PA CMI.
• Medicare billing knowledge and PDPM knowledge.
• Experience with Managed Care and concurrent reviews.
• Background check and drug screen required.

Responsabilités

• Lead Nursing team as RNAC and coordinate MDS assessments.
• Support the Strong Leadership Team and promote quality care.
• Ensure compliance with state/federal regulations and facility policies.
