Job summary

Community Health Resources (CHR) is seeking a part-time Recovery Support Specialist for the Community Support Program in Enfield, CT. The role supports individuals with serious mental illness to live independently by teaching skills, linking to resources, and coordinating care.

You will conduct assessments, develop person-centered treatment plans, and collaborate with family and community supports. A valid Recovery Support Certification or equivalent and a driver’s license are required, along

Requires Recovery Support Certification or education equivalent.
High School Diploma required.

Responsibilities

Conducts Clinical and Risk Assessments, LOCUS, Co-occurring Screens and Functional Assessments.
Collaborates with individuals to create individualized, person centered treatment plans and documents progress.
Use Motivational Interventions to assist clients in meeting their goals and moving along their paths to recovery.
Provide psycho-educational rehabilitation services, group and individual for recovery from psychiatric and substance use disorders.
Assist clients with linking to natural or community supports and resources.
Develop crisis plans (WRAP) and respond to crisis situations when needed.
Provide skills teaching for housing, transportation, nutrition, safety, and daily living.
