Job summary

One Home Health is seeking a compassionate and dedicated Physical Therapist Assistant (PTA) to join our home health team on a part-time basis. This role delivers physical therapy treatments in patients' homes under physician-approved care plans and collaborates with a supportive clinical team.

Applicants must hold an active PTA license in the state, graduate from an APTA-recognized program, and have at least six months of PTA experience.

Qualifications

• Active Physical Therapist Assistant license in the state of employment.
• Graduation from an APTA-recognized PTA program.
• At least six months of PTA experience.
• Strong communication and documentation skills.
• Valid driver's license and ability to travel to patient homes.

Responsibilities

• Deliver PTA treatments and educational support to patients in their homes per care plan.
• Complete visit documentation accurately and submit within timelines.
• Observe and report patient responses, progress, and scheduling changes.
• Collaborate with nurses and other professionals to support care.
• Participate in interdisciplinary meetings and care planning.
• Follow safety, infection prevention, and equipment guidelines.
• Maintain patient privacy and comply with regulations.
• Adhere to agency policies and state requirements.
• Attend staff development and quality improvement activities.
• Perform other related responsibilities as assigned.
