Job summary

Advanced-Health-Care-6 is seeking a Home Health Physical Therapy Assistant (PTA) to support physical therapists in implementing patient care plans within patients' homes. You will work directly with patients to guide exercises, monitor progress, and ensure a safe, effective therapy environment.

Qualifications

• Associate’s degree in Physical Therapy Assistant from an accredited program.
• Valid PTA license in the state of work.
• 1+ years PTA experience, preferably in a home health setting.
• Knowledge of physical therapy principles and best practices.
• Excellent communication and interpersonal skills.
• Ability to prioritize tasks and adapt to changing priorities.
• Ability to build rapport with patients.

Responsibilities

• Assist physical therapists in developing and executing individualized treatment plans.
• Implement exercises, modalities, and interventions as prescribed to improve strength, flexibility, balance, and coordination.
• Educate patients and families on exercises and home exercise programs.
• Monitor and document patient progress and changes in condition.
• Assist with mobility, transfers, and activities of daily living as needed.
• Provide feedback and support to maximize therapeutic outcomes.
• Maintain accurate patient records and documentation.
• Collaborate with therapists and other healthcare professionals for coordinated care.
• Participate in ongoing training and professional development.
• Perform all other duties as assigned.
