Job summary

Gateway Post Acute in Porterville, CA is seeking a compassionate Physical Therapist Assistant (PTA) to join our rehabilitation team on a PRN basis. This part-time position offers flexible scheduling while delivering high-quality patient-centered care in a collaborative rehab environment.

Responsibilities include assisting the Physical Therapist, implementing treatment plans, documenting progress, and supporting residents in achieving mobility and independence per physician orders.

Qualifications

• Graduate of an accredited Physical Therapist Assistant program.
• Skilled Nursing Facility or post-acute experience preferred.
• Current, active California Physical Therapist Assistant license in good standing.
• Must meet all continuing education requirements per state regulations.

Responsibilities

• Treat patients as directed by the supervising Physical Therapist.
• Implement treatment plans in accordance with physician orders and established goals.
• Document daily treatment notes and progress per state and facility requirements.
• Communicate patient progress, concerns, and response to treatment with the supervising PT and interdisciplinary team.
• Participate in patient care conferences, rehab meetings, and discharge planning as needed.
