Job summary

Sequoia Transitional Care in Porterville, CA seeks a compassionate Physical Therapist Assistant (PTA) for a part-time role on our rehabilitation team. Under the supervision of a licensed Physical Therapist, you will implement treatment plans, monitor progress, document care, and collaborate with an interdisciplinary team to help residents regain function.

The ideal candidate has an Associate Degree in Physical Therapist Assistant, is licensed in California, and demonstrates strong communication

Qualifications

• Associate Degree from an accredited PTA program.
• Licensed California PTA in good standing.
• Experience in skilled nursing or rehab settings preferred but not required.
• Strong documentation and computer skills.
• Ability to work collaboratively within an interdisciplinary team.
• Commitment to resident-centered care and safety.

Responsibilities

• Provide PTA services under supervision of a licensed therapist.
• Implement individualized treatment plans and monitor progress.
• Complete timely, accurate documentation per policies and regulations.
• Maintain a safe treatment environment and follow infection control protocols.
• Assist with education of staff and residents on home exercise programs.
• Participate in team meetings and discharge planning as needed.
