Job summary

Lacamas Creek Post Acute is seeking a compassionate Physical Therapist to join our rehabilitation team. You will evaluate patients, develop care plans, and deliver skilled PT services to improve strength, mobility, balance, and independence. You’ll document progress and adjust plans as needed while collaborating with the care team.

The role supports a full-time or PRN schedule in a patient-centered environment that values teamwork and quality outcomes. Licensure or eligibility is required.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Current Physical Therapist license or eligibility for licensure.

Responsibilities

• Evaluate patients and develop individualized plans of care based on their needs and goals.
• Provide skilled physical therapy services to improve strength, mobility, balance, and function.
• Monitor patient progress and adjust treatment plans as appropriate.
• Educate patients, families, and caregivers on exercises and strategies to support progress.
• Accurately document evaluations, treatments, progress, and outcomes.
• Collaborate with Physical Therapist Assistants, nursing, and other care team members.
