Family Service League is seeking a part-time Psychiatric Nurse Practitioner for the Personalized Recovery Oriented Services (PROS) program in Huntington. The Psychiatric Nurse Practitioner will provide psychiatric evaluations, medication management and symptom monitoring to adults with serious mental illness.

The role requires a Master’s degree, NY NP license and DEA credentials, plus at least 3 years of prescribing experience.

Qualifikationen

• Master’s degree from a Nurse Practitioner Program required.
• Current NY State RN and NP license and DEA credentials required.
• Minimum of 3 years prescribing experience; psychiatric prescribing preferred.
• Proficient computer skills; EMR experience preferred.
• Excellent time management and communication skills required.
• Ability to work with a multi-disciplinary team.

Aufgaben

• Understand medical history, prescribe, and interpret lab work and adjust course of treatment as needed.
• Provide psychiatric evaluations and diagnoses for adults.
• The Psychiatric Nurse Practitioner will conduct health assessments of adults with potential acute and/or chronic health problems.
• Initiate referrals to specialty services and foster continuity of care by collaborating with external providers as needed.
• Collaborate with clients’ families and collateral support to provide counseling and education.
• Participate in team meetings and conferences to ensure an interdisciplinary approach to treatment.
• Maintain complete and accurate documentation within the regulatory requirements.
• Identify quality improvement issues and document standards of care and practice.
• Maintain a current level of knowledge relative to professional practice.
• Participate in the evaluation of the cost effectiveness of services provided.
• All other duties as assigned.
