Job summary

HealthDrive is seeking a part-time Primary Care Nurse Practitioner to manage medical needs of skilled nursing residents in sub-acute rehab and long-term care units. The APRN will diagnose, treat, and promote health, collaborating with on-site staff and administrators, serving as primary contact for emergencies and routine visits.

Responsibilities include physical exams, lab interpretation, and coordinating patient health across facilities.

Qualifications

• Master’s degree in nursing or related field.
• Active State NP license.
• Board certification as NP required.
• DEA/Controlled Substance license.
• Valid driver’s license and willingness to travel within territory.

Responsibilities

• Provide primary, post-acute, and preventative care for residents in an assigned facility.
• Order and study diagnostic tests (blood tests, X-rays, etc.).
• Diagnose health conditions and illnesses.
• Propose treatments for chronic and infectious diseases.
• Prescribe medications.
• Maintain accurate records and schedules.
• Inform patients about health conditions and prescribed medications.
• Oversee patients’ overall health.
