Job summary

Advocate Health seeks a Licensed Physical Therapist for the Acute Care Rehabilitation department. The role emphasizes high-quality, compassionate patient care with weekly weekend coverage, orientation, and cross-training opportunities across sites.

Responsibilities include evaluating patients, planning care, and collaborating with physicians while ensuring documentation accuracy and adherence to safety standards. BLS and state licensure are required or in progress.

Qualifications

• Doctorate degree in Physical Therapy from an accredited school.
• Licensed Physical Therapist in the state where you work.
• BLS for Healthcare Providers certification within 90 days unless exempted.

Responsibilities

• Interprets physician referrals and completes musculoskeletal, neuromuscular, cardiopulmonary and/or integumentary evaluations.
• Establishes a plan of care and implements treatment based on evaluation results and functional goals.
• Monitors outcomes, adjusts treatment plans, and communicates with physicians and teammates.
• Completes patient discharge plans including home programs and teaching, and coordinates with community services.
• Maintains documentation, daily stats, billing, and scheduling per department policies and accreditation standards.
• Ensures safety and infection control measures are followed and participates in safety initiatives.
