Job summary

Total Care Therapy is hiring a compassionate Physical Therapist for a part-time role in Fort Wayne, IN. You will evaluate residents, develop treatment plans, and provide skilled PT services in assisted living and independent living settings.

Requirements include a state license to practice as a Physical Therapist, CPR/BLS certification, and a valid driver's license. Experience with geriatrics and home health is preferred but not required. The schedule is Monday-Friday with weekends optional.

Qualifications

• Current State Physical Therapist (PT) License.
• Current CPR/BLS Certification.
• Valid driver's license, reliable transportation, and auto insurance.

Responsibilities

• Evaluate residents and develop individualized plans of care.
• Provide skilled physical therapy treatments, including therapeutic exercise, gait training, balance training, transfer training, and functional mobility interventions.
• Reassess patient progress and modify treatment plans as appropriate.
• Supervise Physical Therapist Assistants (PTAs) in accordance with state regulations.
• Complete timely and accurate documentation using the EMR.
• Educate residents, caregivers, and facility staff on treatment plans and home exercise programs.
