Job summary

Total Care Therapy is seeking a compassionate Physical Therapist to join our growing team in Schererville, IN. You will evaluate, develop, and implement individualized treatment plans to improve mobility, restore function, reduce pain, and maintain independence for residents.

Qualifications

Current State Physical Therapist (PT) license.
CPR/BLS certification active.
Valid driver's license with reliable transportation.
Experience in geriatrics, home health, assisted living, or skilled nursing preferred.

Responsibilities

Evaluate residents and develop individualized plans of care.
Provide skilled PT treatments including exercises, gait and balance training.
Reassess progress and modify treatment plans as needed.
Supervise PTAs per state regulations.
Document timely in EMR and educate residents and staff.
Collaborate with interdisciplinary team for coordinated care.
Maintain a productive clinical caseload with quality therapy.
