Job summary

Total Care Therapy in Granger, Indiana, is seeking a compassionate Physical Therapist to evaluate residents and implement individualized treatment plans that improve mobility, reduce pain, and maintain independence in older adults. This full-time role is on site within assisted living and independent living communities.

Qualifications

Current State Physical Therapist (PT) license is required.
Current CPR/BLS certification is required.
Valid driver's license and reliable transportation.

Responsibilities

Evaluate residents and develop individualized plans of care.
Provide skilled physical therapy treatments including exercises, gait, balance, and mobility training.
Reassess progress and modify treatment plans as needed.
Supervise PTAs per state regulations.
Document care using EMR systems.
Educate residents and staff on treatment plans and home programs.
Collaborate with interdisciplinary team for coordinated care.
