Job summary

FOX Rehabilitation is seeking licensed Physical Therapists for Fort Wayne, IN to deliver one-on-one geriatric care in patients' homes or within senior living communities. The role emphasizes autonomy, flexible scheduling, and meaningful outcomes, with the option to incorporate telehealth into treatment plans.

Qualifications

• Valid Physical Therapy license in the state(s) of practice, or eligibility to obtain
• Degree from an accredited Physical Therapy program
• Basic computer and EMR proficiency
• Current CPR certification

Responsibilities

• Provide one-on-one physical therapy to older adults wherever they call home (private residences, senior living communities, etc.)
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment
• Deliver therapy sessions that support wellness, longevity, mobility, and independence
• Document effectively within FOX’s EMR system per regulatory standards
• Collaborate with interdisciplinary team (PTs, OTs, SLPs, Fitness Specialists)
• Educate patients, caregivers, and staff to promote safety and aging in place
• Provide telehealth/virtual care when appropriate to extend access
