Job summary

CommuniCare Rehab, a member of the CommuniCare Family of Companies, has a part-time Physical Therapist position at Cameron Healthcare Center in Cameron, WV. Therapists will evaluate residents, develop and supervise plans of care, and coordinate with the in-house rehab team.

Qualifications

• A PT degree from an accredited program.
• Active or pending State Physical Therapy license.

Responsibilities

• Evaluate, review, and screen residents' medical charts and develop treatment plans.
• Implement treatment plans through direct care and supervise PTA activities.
• Lead interdisciplinary meetings and educate the rehab team and families.

PHYSICAL THERAPIST- Part Time

Qualifications:

• A Bachelor’s, Master’s, or Doctorate Degree in Physical Therapy from an Accredited Program.

• Hold a valid or pending State Physical Therapy License

Job Responsibilities include, but are not limited to:

• Evaluate, Review, and Screen patient’s/resident’s medical charts, evaluations under physician order and develop a treatment plan in accordance with patient’s needs.

• Implement treatment plan through direct treatment, as well as education of the plan of care to all team members, including providing necessary supervision for PTAs.

• Attend/Lead interdisciplinary meetings to relay clinical findings and make recommendations to other members of rehab team, as well as conducting staff/family training and education when appropriate.
