Job summary

Total Care Therapy is seeking a compassionate Physical Therapist to join our growing Fort Wayne team. You will evaluate residents, develop individualized plans, and provide skilled PT services to improve mobility and independence in assisted living communities.

We offer per-visit compensation based on experience, part-time scheduling with weekday hours, and opportunities for mentorship within a therapist-owned company. Fort Wayne North and West locations available, with weekend options optional.

Qualifications

• Current State PT License
• Current CPR/BLS Certification
• Valid driver’s license and reliable transportation
• Experience in geriatrics, home health, assisted living, or skilled nursing preferred but not required

Responsibilities

• Evaluate residents and develop individualized plans of care.
• Provide skilled physical therapy treatments including therapeutic exercise, gait training, balance training, transfer training, and functional mobility interventions.
• Reassess patient progress and modify treatment plans as appropriate.
• Supervise Physical Therapist Assistants (PTAs) per state regulations.
• Complete timely and accurate documentation using the EMR.
• Educate residents, caregivers, and facility staff on treatment plans and home exercise programs.
• Collaborate with interdisciplinary team for coordinated, patient-centered care.
• Maintain a productive clinical caseload while delivering exceptional therapy services.
