Job summary

Healthcare Therapy Services, Inc. in Fort Wayne, IN is seeking a full-time Physical Therapist to join a therapist-led team at a CMS 5-Star continuing care retirement community on the northeast side of town.

The role emphasizes autonomy, collaboration, and clinical excellence in a supportive environment. The successful candidate will hold an active Indiana PT license or be eligible to obtain one, with an advanced degree and successful licensure, and will work Monday–Friday, 30–40 hours per week

Qualifications

• Active Indiana Physical Therapist license or eligibility to obtain one.
• Advanced degree from an ACOTE-accredited physical therapy program.
• Pass the Indiana licensure exam and meet licensure requirements.

Responsibilities

• Provide physical therapy across short-term rehab, long-term care, outpatient, memory care, and assisted living.
• Collaborate with an interdisciplinary team to develop patient-centered plans.
• Deliver evidence-based therapy focused on improving quality of life for older adults.
