Memorial Home Services in Jacksonville, IL is seeking a part-time Physical Therapist to provide and coordinate comprehensive patient care. You must hold a PT degree and IL license (or license pending with ATT) and CPR certification.

Qualifications

• Graduate of an approved school of Physical Therapy.
• Basic knowledge in physical therapy practices in accordance with the P.T. Practice Act.
• CPR certification is required.

Responsibilities

• Provide and coordinate comprehensive PT care for patients of all ages through evaluation, planning and treatment implementation.
• Ensure compliance with established standards, policies, procedures, protocols and guidelines.
• Schedule and coordinate PRN patient care as needed.
