Job summary

The position requires a Bachelor’s degree in Physical Therapy, Illinois licensure (or license pending) and CPR certification. Per diem schedule with competitive daily rates, supporting the hospital’s rehabilitation team and collaboration with physicians.

Qualifications

• Bachelor in Physical Therapy or related required.
• Must be licensed in Illinois or license pending and CPR card.

Responsibilities

• Provide and coordinate comprehensive physical therapy care per plan.
• Perform evaluations, develop and adjust treatment plans to progress toward goals.
• Document daily notes and communicate with physicians; supervise students and aides.
• Collaborate with rehabilitation team and participate in program improvements.
