Job summary

Concept Rehab, Inc. is seeking a compassionate Part-time Physical Therapist to join our rehabilitation team in a skilled nursing and post-acute care setting in Ohio.

You will work with a multidisciplinary team to help patients regain strength, mobility and independence after illness, injury, or hospitalization. The ideal candidate is a graduate of an accredited PT program, holds an active Ohio license, and demonstrates strong communication, clinical reasoning, and patient education skills.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Current Ohio Physical Therapist license in good standing.
• Experience in skilled nursing, post-acute, or geriatric rehabilitation preferred.
• Excellent communication and clinical reasoning skills.
• Ability to build strong relationships with patients, families, and healthcare professionals.
• New graduates are encouraged to apply.

Responsibilities

• Perform comprehensive physical therapy evaluations and establish individualized treatment plans.
• Deliver evidence-based interventions focused on mobility, balance, strength, pain management, and functional independence.
• Monitor patient progress and adjust treatment strategies to maximize outcomes.
• Collaborate with physicians, nursing staff, families, and interdisciplinary care teams.
• Provide supervision and direction to Physical Therapy Assistants in accordance with state regulations.
• Complete timely, accurate, and compliant clinical documentation.
• Educate patients and caregivers on safe mobility techniques and discharge recommendations.
• Participate in care conferences, team meetings, and quality improvement initiatives.
• Promote a positive patient experience through compassionate and patient-centered care.
