Job summary

Concept Rehab, Inc in Centerville, OH is seeking a compassionate Physical Therapist to join our skilled nursing and post-acute rehab team. You will perform evaluations, develop individualized plans, and deliver evidence-based therapies to restore mobility and independence.

You will collaborate with physicians, nursing staff, and families, supervise PT aides as appropriate, and participate in quality improvement initiatives.

Qualifications

• Must have graduated from an accredited physical therapy program.
• Current Ohio Physical Therapist license in good standing.
• Experience in skilled nursing, post-acute, or geriatric rehab preferred.
• Excellent communication and clinical reasoning skills.
• Ability to build strong relationships with patients, families, and healthcare professionals.
• New graduates are encouraged to apply.

Responsibilities

• Perform comprehensive physical therapy evaluations and establish individualized treatment plans
• Deliver evidence-based interventions focused on mobility, balance, strength, pain management, and functional independence
• Monitor patient progress and adjust treatment strategies to maximize outcomes
• Collaborate with physicians, nursing staff, families, and interdisciplinary care teams
• Provide supervision and direction to Physical Therapy Assistants in accordance with state regulations
• Complete timely, accurate, and compliant clinical documentation
• Educate patients and caregivers on safe mobility techniques and discharge recommendations
• Participate in care conferences, team meetings, and quality improvement initiatives
• Promote a positive patient experience through compassionate and patient-centered care
