Job summary

Concept Rehab is seeking a Part-Time Physical Therapist (PT) to join our team in a Skilled Nursing Facility in Delaware, Ohio. This role provides post-acute rehabilitation, skilled nursing care, and long-term care services for adult and geriatric patients.

You will evaluate and treat patients recovering from illness, injury, surgery, hospitalization, and functional decline, collaborating with physicians, nursing staff, and families to create individualized plans of care that improve mobility,

Qualifications

• Bachelor's, Master's, or Doctorate in Physical Therapy.
• Current Ohio Physical Therapist license in good standing.
• Strong clinical reasoning and patient care skills.
• Excellent communication and documentation abilities.
• Ability to work collaboratively within an interdisciplinary rehabilitation team.

Responsibilities

• Perform physical therapy evaluations within established timelines following physician referral.
• Develop individualized plans of care based on patient needs and goals.
• Provide evidence-based physical therapy treatment interventions.
• Monitor patient progress and modify treatment plans as clinically appropriate.
• Complete discharge planning and recommendations.
