Job summary

Leisure Glen Post Acute Care Center in Glendale, California is seeking a Part-Time licensed Physical Therapist to join our Rehab Team. You will provide skilled physical therapy interventions to a diverse elderly population, focusing on mobility and independence in a post-acute setting.

As part of a collaborative care team, you will assess residents, develop individualized treatment plans, document progress, and educate residents and families on home exercise programs.

Qualifications

• Licensed Physical Therapist in California.
• Minimum of 2 years of healthcare experience, preferably in a skilled nursing facility.
• Knowledge of current physical therapy practice standards and regulations.

Responsibilities

• Evaluate residents’ physical abilities, functional limitations, and mobility needs using standardized assessments
• Develop and implement individualized treatment plans based on assessment results, resident goals, and physician orders
• Provide skilled physical therapy interventions, including therapeutic exercise, manual therapy, and modalities
• Monitor and document resident progress and adjust treatment plans as necessary
• Communicate with interdisciplinary team members, including physicians, nurses, occupational therapists, and speech therapists, to coordinate care and ensure optimal outcomes for residents
• Educate residents and family members on physical therapy goals, treatment plans, and home exercise programs
• Maintain accurate and timely documentation of physical therapy services provided
• Participate in team meetings, in-service education, and quality improvement initiatives
• Maintain current knowledge of physical therapy practice standards and regulations
