Job summary
Leisure Glen Post Acute Care Center in Glendale, California, is looking for a motivated and compassionate Physical Therapist (PT) to join our rehabilitation team.
We offer full-time and per diem opportunities, with weekend availability highly desirable. There is ample on-site parking, a welcoming work environment, and a strong, supportive rehab team.
Experience is valued but new graduates are welcome to apply, and candidates must hold a California PT license.
Qualifications
- Current California Physical Therapist license.
- Strong clinical reasoning, communication, and teamwork skills.

- Weekend availability is highly desirable.
Responsibilities
- Evaluate and treat residents with a variety of orthopedic, neurological, and functional mobility needs.
- Develop individualized treatment plans to improve strength, balance, mobility, and independence.
- Provide skilled physical therapy interventions to support rehabilitation and recovery.
- Complete accurate and timely evaluations, progress reports, and treatment documentation.
- Collaborate with our rehabilitation team, nursing staff, and other interdisciplinary team members.
- Educate residents and families on mobility, safety, fall prevention, and therapeutic exercises.
- Participate in discharge planning and interdisciplinary care meetings.
