Qualifikationen

• Graduate of an accredited Physical Therapy program.
• Current Pennsylvania PT license or eligibility for licensure.
• Experience in SNF/LTC settings preferred but not required.
• Strong communication, organizational, and interpersonal skills.
• Compassionate, patient-centered approach to care.
• Ability to work independently within a collaborative rehabilitation team.
• Familiarity with Net Health and PCC is a plus.

Aufgaben

• Evaluate residents and develop individualized physical therapy treatment plans.
• Provide skilled physical therapy services to improve mobility, strength, balance, and independence.
• Assist residents in achieving their highest possible level of independence and quality of life.
• Collaborate with nursing staff, physicians, OT, SLP, and other healthcare professionals for coordinated care.
• Maintain accurate and timely clinical documentation using Net Health.
• Participate in interdisciplinary care planning and resident progress discussions.
• Educate residents, families, and caregivers on therapeutic exercises, mobility techniques, and safety strategies.
• Ensure compliance with all state, federal, and facility regulations.
