Job summary

Senior LIFE PA in York, PA is seeking a Part-Time Physical Therapist to join an interdisciplinary team. New grads are encouraged to apply and you will work weekdays with a daylight schedule in an all-inclusive home- and community-based care setting.

Qualifications

• Must be licensed to practice physical therapy in Pennsylvania.
• Relevant degree from an accredited college or university.
• At least one year of experience working with elderly/geriatric population.

Responsibilities

• Evaluating members’ mobility levels and devising approaches to improve strength, ROM, and motor control.
• Developing treatment programs and setting goals with periodic progress checks.
• Assessing home environments to minimize hazards and provide in-home support as needed.
• Documenting changes in care plans and participating in individual therapies and group activities.
• Collaborating with physicians, nurses, and other caregivers to update plans of care.
• Completing fall risk assessments and developing prevention plans.
