Reid Health Main Campus is seeking a pharmacist to join our team on a part-time basis. The role covers 32 hours weekly with a variable weekend schedule, generally 8-hour shifts, including one weekend per month.

Responsibilities include clinical checking, provider communications, patient counseling, and participation in QA&I and antimicrobial stewardship. The ideal candidate will hold a Bachelor of Pharmacy or PharmD and maintain an active Indiana pharmacist license, with flexible availability

Competenze

Bachelor of Pharmacy degree is required.
PharmD degree from ACPE-accredited college is preferred.
Active Indiana pharmacist license is required.

Mansioni

Reviews and interprets provider orders and performs clinical checking.
Communicates clinical concerns such as allergies, drug interactions, side effects, and min/max warnings to providers.
Monitors patient medication therapies and provides interventions.
Approves all medications and IVs before they are dispensed.
Counsels patients and caregivers on medication use.
Documents all clinical activities.
Participates in QA&I or antimicrobial stewardship activities as applicable.
Reports medication errors, both prevented and actual.
