Job summary

Reid Health Main Campus is seeking a pharmacist to join our team on a part-time basis. The role covers 32 hours weekly with a variable weekend schedule, generally 8-hour shifts, including one weekend per month.

Responsibilities include clinical checking, provider communications, patient counseling, and participation in QA&I and antimicrobial stewardship. The ideal candidate will hold a Bachelor of Pharmacy or PharmD and maintain an active Indiana pharmacist license, with flexible availability

Qualifications

• Bachelor of Pharmacy degree is required.
• PharmD degree from ACPE-accredited college is preferred.
• Active Indiana pharmacist license is required.

Responsibilities

• Reviews and interprets provider orders and performs clinical checking.
• Communicates clinical concerns such as allergies, drug interactions, side effects, and min/max warnings to providers.
• Monitors patient medication therapies and provides interventions.
• Approves all medications and IVs before they are dispensed.
• Counsels patients and caregivers on medication use.
• Documents all clinical activities.
• Participates in QA&I or antimicrobial stewardship activities as applicable.
• Reports medication errors, both prevented and actual.
