Qualifications

Responsibilities

• Assist nursing staff with delegated patient care tasks and keep environment safe and organized
• Collect and record patient data (vital signs, height, weight, intake/output) and report to RN/LPN
• Assist with meals, bathing, grooming, oral care, linen changes, and mobility aid use
• Assist with patient positioning, ambulation, and use of walkers/crutches/canes/wheelchairs
• Collect urine/stool samples and perform glucose monitoring; document results
• Communicate patient information effectively to care team and maintain professionalism
• Maintain infection control and PPE protocols and a clean environment
• Participate in performance improvement and risk management activities
• Maintain regular attendance and join in on policy adherence
