Job summary

Nantucket Cottage Hospital, part of Mass General Brigham, is seeking a Palliative Care Nurse Specialist to provide direct patient care and clinical management for patients and families. The Nurse works with the Nurse Practitioner Director of PASCON and a multidisciplinary team to ensure coordinated, compassionate care across hospital, home, and nursing home settings.

Qualifications include a Bachelor's Degree in Nursing (Master's preferred) and MA RN license.

Qualifications

• Bachelor's Degree in Nursing is required; a Master's Degree is preferred.
• MA RN license is required.
• Nursing experience: 2–3 years required; hospice/palliative care 2–3 years preferred.

Responsibilities

• Organize, develop, plan, implement, and evaluate Palliative & Supportive Care Program goals and procedures.
• Provide high quality direct patient care, including education and counseling, in any setting.
• Record patient history and physical exams and discuss findings with NP Manager and physician.
• Enhance continuity of care in collaboration with NP manager and physician.
• Perform and oversee specialized procedures with collaborating physician.
