Job summary

Nantucket Cottage Hospital is seeking a Palliative Care Nurse Specialist to provide direct patient care and clinical management on a 20-hour-per-week day shift.

You will collaborate with the Nurse Practitioner Director of PASCON, physicians, and a multidisciplinary team to deliver comprehensive palliative care across hospital, home, and nursing facilities, ensuring smooth transitions for patients and families.

Qualifications

• Bachelor's degree required; Master's preferred.
• MA RN license required.
• Nursing experience 2–3 years; hospice/palliative care 2–3 years preferred.

Responsibilities

• Organize, develop, plan, implement, and evaluate program goals and procedures for Palliative & Supportive Care.
• Provide high quality direct patient care, including education and counseling in any setting.
• Record patient history and exam findings; discuss with NP Manager and physician as needed.
• Manage patient care in collaboration with NP manager and physician to enhance continuity.
• Perform and oversee specialized procedures as agreed with collaborating physician.
