Mass General Brigham is seeking an RN Navigator to support the Outpatient Palliative Care Clinic. You will coordinate care for patients with serious or complex illness, triage referrals, and work with clinicians to ensure timely intervention and appropriate level of care across the continuum.

You will educate clinicians and staff on pathway criteria, facilitate patient entry into designated programs, and monitor progression to ensure optimal outcomes.

Formación

• Bachelor's Degree in Nursing required.
• MA Registered Nurse License required.
• BLS Certification required.

Responsabilidades

• Collaborate with interdisciplinary team to create tailored care plans.
• Develop plans with the MD for appropriate services for patients.
• Report observations to appropriate care team members.
• Coordinate discharge planning with the healthcare team.
• Address patient problems using multidisciplinary strategies.
• Educate patients on navigating the health system.
