Compass Regional Hospice in Centreville, MD is seeking a Part-time Palliative Care Nurse Practitioner to provide advanced clinical assessments, symptom management, and coordination of care for hospice patients. This on-site role requires collaboration with the Medical Director and patient physicians.

The NP will conduct home and facility visits as needed, maintain EMR records, participate in IDG meetings, and ensure adherence to ethical, regulatory standards.

Formación

• Must hold a current advanced practice license with appropriate certification in the State of Maryland.
• Advanced clinical assessment and pain management skills with knowledge of current palliative medicine standards.
• Demonstrates critical thinking and good judgment in emergency situations.
• Willingness to complete Hospice and Palliative Care Certification within 24 months.

Responsabilidades

• Provide comprehensive health assessments and prescribe treatments under protocol.
• Perform Face-to-Face visits for certification and home-based patient care as needed.
• Attend Palliative IDG meetings and coordinate care with the team.
• Maintain up-to-date EMR documentation of patient plans and changes.
• Ensure documentation complies with agency guidelines and regulatory requirements.
• Collaborate with external referral sources to increase access to hospice services.
