Job summary
AccentCare, Inc. is seeking a Nurse Practitioner in Palliative Care for a part-time, PRN role covering Loveland and surrounding areas. The position supports a 8:00am - 5:00pm schedule with 24 hours weekly, focusing on comprehensive symptom management and quality end-of-life care.
The role requires a registered NP, state licensure, CPR certification, and a valid DEA license where care is provided. Travel to multiple locations is expected to support patients in the community.
Qualifications
- Graduated from an approved school of professional nursing.
- One year experience as a Nurse Practitioner is preferred.
- Licensed to practice as a Nurse Practitioner in the state of operation.
- Active DEA license in the state where care is provided.
- CPR certification preferred.
- Must be a licensed driver who can travel to all business locations.
Responsibilities
- Develop and implement clinical plans of care for patients with chronic and terminal illnesses.
- Perform patient assessment, history, and physical; provide recommendations on symptom management.
- Monitor patient progress and communicate with Medical Director and team; assist with discharge planning and family education.
- Maintain communication with supervising and attending physicians to review patient records and care plans.
- Coordinate care with other Palliative Care staff and agencies as needed to optimize patient/family outcomes.
- May provide support at time of death.
- Prescribe medications as permitted by state regulations.
