AccentCare Inc. in Seaford, DE is seeking a Nurse Practitioner (Palliative Care) for a PRN/Part-Time role. You will assess patients with chronic and terminal illnesses, develop care plans and coordinate symptom management in a home health oriented setting.

The position requires graduation from an approved nursing program with NP licensure and current CPR; DEA license preferred; travel to multiple locations; compensation $55-$62 per hour with flexible 8-10 hours weekly.

Qualifications

• Graduated from an approved school of professional nursing.
• One year experience as a Nurse Practitioner is preferred.
• Licensed to practice as a Nurse Practitioner in the state of agency operation.
• Active DEA license in the state where care is being provided.
• Possess and maintain CPR certification; travel to all business locations is required.

Responsibilities

• Develop and implement clinical plans of care for patients with chronic and terminal illnesses.
• Perform patient assessment, history and physical; recommend symptom-management strategies.
• Monitor patient progress and update team leaders; collaborate on discharge planning and patient/family education.
• Maintain communication with supervisory and attending physicians to review patient records and care plans.
• Prescribe medications as permitted by state regulations and participate in continuum of care for hospice-appropriate patients.
