Job summary

AccentCare, Inc. in Seaford, DE is seeking a Nurse Practitioner for palliative care on a PRN, part-time basis. This on-site role supports patients with chronic and terminal illnesses with comprehensive care plans and symptom management.

Ideal candidates hold an approved NP license for Delaware, possess or pursue a DEA license, and have CPR certification. Travel to multiple locations may be required, with a competitive hourly salary and benefits package.

Qualifications

• Graduated from an approved school of professional nursing.
• One year experience as a Nurse Practitioner is preferred.
• Licensed to practice as a Nurse Practitioner in the state of agency operation.
• Active DEA license in the state where care is being provided.
• Possess and maintain valid CPR certification while employed in a clinical role, preferred.
• Must be a licensed driver who can travel to all business locations.
• Meet the regulations and requirements of the state(s) in which program provides services.

Responsibilities

• As a Palliative Care Nurse Practitioner, you will develop and implement clinical plans of care for patients with chronic and terminal illnesses.
• Perform patient assessment, history and physical; recommendations on aggressive symptom-management.
• Monitor patient progress; update the Medical Director, Team Director and team members; consult regarding discharge planning; and patient/family teaching.
• Maintain communication with the supervisory physician(s) and attending physician to review patient records and care plans on a routine basis.
• Initiate communication with attending physicians, other Palliative Care staff members and other agencies as needed to coordinate optimal care and use of resources for patient/family.
• May provide support at time of death.
• Prescribe medication as permitted by the state and/or the State Board of Nursing.
• Participate in continuum of care for patients that are hospice appropriate and ongoing education programs.
