Qualifications

• Education: Graduate of an accredited physical therapy program; Doctorate degree preferred.
• Licensure/Certification: Current Ohio PT license; BLS certification.
• Experience: One year of clinical experience preferred.
• Communication: Effective communication skills.
• Ability: Ability to travel to other sites as necessary.

Responsibilities

• Performs physical therapy evaluation, establishes and implements treatment plan for patients referred by a provider and assigned by the manager, clinical coordinator or designee.
• Documents PT evaluation results, treatment interventions, progress toward goals, and discharge records.
• Maintains proper billing for the services provided.
• Delegates treatment procedures to the Physical Therapist Assistant/PTA and reviews their documentation.
• Communicates with other health care providers on patient treatment, continuity of care, and discharge planning.
