Job summary

Southern New Hampshire Health is seeking a part-time Physical Therapist to provide outpatient and inpatient services to a diverse patient population in the Nashua area. You will evaluate patients, develop and modify treatment plans, educate patients and families, and document in the EMR.

This role requires NH PT license and CPR certification, and prior outpatient PT experience is preferred. Shift is 20 hours per week, with Monday 9am-4pm, Wednesday 11:30am-7pm, Friday 9am-4pm; part-time benefits

Qualifications

• G raduate of an accredited Physical Therapy program.
• Current (or pending) New Hampshire Physical Therapy license required.
• CPR Certification.
• Experience in outpatient PT preferred (1–2 years).
• Strong time-management, communication, and documentation skills.
• Ability to provide safe patient handling and demonstrate appropriate physical and clinical judgment.

Responsibilities

• Evaluate patients using subjective data, objective measures, clinical assessment, prognosis, and function-based goals, establishing effective and individualized treatment plans.
• Implement and modify patient-specific treatment programs to support functional progress and long-term outcomes.
• Develop and deliver patient and family education, adjusting strategies as needed to maximize engagement and understanding.
• Document all clinical findings, plans, and progress in the electronic medical record in a timely and accurate manner.
• Complete all required insurance and third-party payer information.
• Provide clinical supervision to new staff members and/or affiliating students as assigned.
• Collaborate with physicians, care teams, and support staff to ensure coordinated and effective treatment.
• Maintain clinical competency and utilize equipment and techniques appropriate to training and experience.
• Demonstrate excellent customer service and support a safe and positive patient experience.
