Southern New Hampshire Medical Center, part of SolutionHealth, is seeking a Physical Therapist to join our outpatient/inpatient team in Nashua. You will evaluate patients of all ages and design individualized treatment plans to restore mobility and function.

We value collaborative care, ongoing education, and patient-centered approaches. Current NH PT license required; CPR certification and experience in outpatient PT preferred.

Formación

• Graduate of an accredited Physical Therapy program.
• Current New Hampshire Physical Therapy license required (or pending).
• CPR Certification is required.

Responsabilidades

• Evaluate patients using subjective data, objective measures, prognosis, and goals to establish treatment plans.
• Implement and modify patient-specific treatment programs to support functional progress.
• Educate patients and families to maximize engagement and understanding.
• Document all clinical findings and progress in the EMR timely and accurately.
• Complete insurance and third-party payer information.
• Provide clinical supervision to new staff or students as assigned.
• Collaborate with physicians and care teams for coordinated treatment.
