Job summary

Southern New Hampshire Health is seeking a Physical Therapist to provide outpatient and inpatient services to a diverse age range in Nashua, NH. The role includes evaluating, treating, educating, and documenting patient progress with an emphasis on evidence-based practice.

Qualified candidates will have an accredited PT degree, NH license (or pending), CPR, and experience with Graston, lymphedema therapy, and dry needling.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Current NH Physical Therapy license or pending licensure required.
• CPR certification required.

Responsibilities

• Evaluate patients using subjective data, objective measures, clinical assessment, prognosis, and function-based goals, establishing effective and individualized treatment plans.
• Implement and modify patient-specific treatment programs to support functional progress and long-term outcomes.
• Develop and deliver patient and family education, adjusting strategies to maximize engagement and understanding.
• Document all clinical findings, plans, and progress in the EMR in a timely and accurate manner.
• Complete all required insurance and third-party payer information.
• Provide clinical supervision to new staff members and/or affiliating students as assigned.
• Collaborate with physicians, care teams, and support staff to ensure coordinated and effective treatment.
• Maintain clinical competency and utilize equipment and techniques appropriate to training and experience.
• Demonstrate excellent customer service and support a safe and positive patient experience.
