Job summary

Southern New Hampshire Health is seeking a full-time Physical Therapist to deliver outpatient rehab services in Nashua, NH. The role includes evaluating patients, creating individualized treatment plans, and educating patients and families for optimal functional outcomes.

Candidates should have a PT degree, NH license (or pending), and CPR. Outpatient experience and additional certifications in Graston, Lymphedema, or Dry Needling are a plus.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Licensure: NH Physical Therapy license (current or pending).
• CPR Certification.

Responsibilities

• Evaluate patients using subjective data, objective measures, prognosis, and function-based goals.
• Implement and modify patient-specific treatment programs to support functional progress.
• Develop and deliver patient and family education, adjusting strategies as needed to maximize engagement.
• Document all clinical findings, plans, and progress in the electronic medical record in a timely and accurate manner.
• Complete all required insurance and third-party payer information.
• Provide clinical supervision to new staff members and/or affiliating students as assigned.
• Collaborate with physicians, care teams, and support staff to ensure coordinated and effective treatment.
• Maintain clinical competency and utilize equipment and techniques appropriate to training and experience.
• Demonstrate excellent customer service and support a safe and positive patient experience.
