Job summary

The Kensington Sierra Madre in Sierra Madre, California is seeking a Part-Time Occupational Therapist to join Prava Therapy’s embedded team. You will provide skilled OT within the community, working with residents, families, caregivers, and staff to support independence and meaningful daily activities.

This role emphasizes functional performance in everyday life, collaboration with the interdisciplinary team, and the use of innovative tools to measure meaningful outcomes within a community

Qualifications

• OT license valid in California.
• Experience in community-based or embedded therapy preferred.

Responsibilities

• Provide skilled occupational therapy tailored to each resident's goals, routines, and daily activities.
• Evaluate and treat residents with functional, cognitive, and physical needs.
• Develop care plans focused on participation, independence, safety, and quality of life.
• Address ADLs/IADLs, cognition, balance, mobility, and environmental barriers as appropriate.
• Help residents participate in meaningful daily activities and routines.
• Communicate changes in function proactively with the interdisciplinary team.
• Collaborate with caregivers and staff to reinforce therapy strategies outside sessions.
• Educate families and staff to support greater independence.
• Build strong relationships with residents and families.
• Collaborate with the Prava Therapy team and community care team.
• Document using Prava's technology and AI-enabled tools thoughtfully and efficiently.
• Measure outcomes that matter beyond minutes or visits.
• Contribute to a therapy culture that is visible, responsive, and integrated into the community.
