Job summary

The Kensington Sierra Madre by Prava Therapy is seeking a Part-Time Occupational Therapist to join an embedded therapy team in Sierra Madre, CA. You will work with residents, families, caregivers, and staff to promote independence, participation, and meaningful daily routines within the community.

You will provide skilled OT, assess a range of functional needs, and develop plans focused on safety, participation, and quality of life.

Qualifications

• Licensed OT in California (or eligible)
• Experience in community-based therapy
• Ability to work within an embedded therapy model

Responsibilities

• Provide skilled occupational therapy tailored to each resident's goals and routines.
• Evaluate and treat residents with functional, cognitive, and mobility needs.
• Develop meaningful plans of care focused on participation, safety, and quality of life.
• Address ADLs, IADLs, cognition, balance, and environment to support independence.
• Collaborate with caregivers, nursing, and leadership to reinforce strategies beyond sessions.
• Document thoughtfully using Prava's technology and AI-enabled tools.
• Measure outcomes that matter to residents and families.
