CarePlus New Jersey is seeking a Group Facilitator for its Partial Care Program in Paramus, NJ. You will work with individuals diagnosed with severe mental illness to develop coping skills, assist with symptom management, and improve socialization and daily functioning.

Qualifications

• Bachelor's Degree in Mental Health discipline required (Psychology, Social Work, Human Services, Counseling, Psychiatric Rehabilitation or Criminal Justice).
• 1 year of verified related work or life experience in mental health desirable
• Valid driver's license insurable by the agency's automobile insurance.

Responsibilities

• Manage the provision and coordination of program services
• Prepare a complete written intake assessment which includes treatment plans, relevant goals, behaviorally-measurable goals that serve as indexes for evaluating treatment progress, and realistic time frames for achievement.
• Perform ongoing assessment of consumers by exhibiting knowledge of the age-specific populations, developmental levels, and diagnosis.
• Run group sessions with clients.
• Provide verbal counseling and crisis intervention services.
• Help with the acquisition of transportation and safely transport consumers daily.
