Job summary

Theoria Medical is seeking a part-time Medical Director and Attending Physician to provide care in local skilled nursing facilities. You will build lasting relationships with residents, focusing on continuity and a patient-centered approach.

Qualifications

• MD/DO degree with active U.S. medical license required.
• Board certification in Internal Medicine, Family Medicine, Emergency Medicine, Geriatrics or related field preferred; eligible for licensure.
• Part-time schedule with attending physician responsibilities at SNFs.

Responsibilities

• Serve as attending physician for patients in assigned SNFs.
• Collaborate with NPs/PAs for daily patient care and supervision.
• Provide high-quality primary care, diagnosing and treating acute and chronic conditions.
• Participate in quality initiatives including chronic care management and remote monitoring.
